Plain-language guide

Cautions desk

# What needs attention before you begin?

Bring your health history into the conversation.

Write down past health problems, current medicines, pregnancy plans, and any planned operation. Ask how to reach the clinician if something changes during care.

Question to keep

What matters in my history, and when is help urgent?

## For severe allergic signs

If breathing is hard or the face or tongue swells, a severe allergic reaction may be the cause.

For those severe signs, go to the nearest emergency room or call 911 now.

Side effect

## An unwanted effect of a medicine

The semaglutide (Wegovy) shot label was checked in September 2026. It lists nausea, which means feeling sick.

Vomiting, loose stools, and stools that are hard to pass are also common.

Stomach problems can be severe. Losing too much fluid can harm the kidneys.

Severe belly pain that lasts may spread to the back. The label says this can be pancreatitis: painful swelling in a body part inside the belly called the pancreas.

Get medical help for that pain. A symptom list cannot tell you its cause.

Ask it plainly

When is a call enough, and which problems call for urgent care?

Boxed warning

## A caution set apart on the label

The small body part in your neck is called the thyroid. As of September 2026, semaglutide labels carried a boxed warning about thyroid tumors found in rats and mice. A boxed warning is the strongest kind of warning on a medicine label.

Whether semaglutide causes these tumors in people remains unknown.

Medullary thyroid cancer is one rare type of thyroid cancer. Keep that exact name when asking about your family's history. The label rules out semaglutide when that cancer has occurred in you or someone in your family.

It also rules out use for people with MEN 2, a rare condition inherited within families. A past serious allergic reaction to semaglutide or its other ingredients also rules out use.

Ask it plainly

Does anything in my own or my family's health history rule this medicine out?

Health history

## Details the clinician needs to hear

The label asks about pregnancy plans and other diabetes medicines. It also asks about eye damage from diabetes, planned surgery, and severe problems with food leaving the stomach.

The label advises against semaglutide (Wegovy) for people with severe gastroparesis. This means food leaves the stomach very slowly.

Tell the care team about semaglutide before an operation or a procedure with anesthesia. Anesthesia is medicine used to prevent pain or awareness during the procedure.

The label reports rare cases of stomach contents entering the lungs during those procedures. The evidence does not show whether stopping the drug first would reduce that risk.

Ask it plainly

How does my history change the plan, including any planned procedure?

Cold storage

## A delivery problem needs an answer

The Food and Drug Administration (FDA) says not to use a shot meant to stay cold if it arrives warm or was not kept cold enough. That can affect its quality.

FDA has received reports of mistakes with compounded semaglutide, medicine prepared to order. Some reports involved hospital care.

Some mistakes may involve measuring or working out the amount. Reports are not proof that the drug caused the problem.

Ask it plainly

Who will address a delivery problem or unclear instructions before I use the medicine?

Keep this thought

Take your health history and your questions to the clinician. Keep the plan for getting help somewhere you can find it quickly.
